Objectives
- Hear family caregivers' perspectives on strengthening a palliative approach
- Learn about evidence-informed practices to strengthen family-centred care
- Imagine what a care conference designed by residents and family caregivers would look like
- Adapt care conferences to address significant changes in health, including near the end of life
- Review practices to strengthen bereavement care
- Provide input on strengthening relationships with family caregivers
- Access resources to support family-centred care in long-term care
